  • DS-156: Non-Immigrant Visa Application
  • DS-156K (Use with Form DS-156): Non-Immigrant Fiance(e) Visa Application
  • DS-157: Supplemental Non-Immigrant Visa Application
  • I-134: Affidavit of Support
